Our 24/7 physician-led telehealth service, PHP Telehealth, is there to support after hours, weekends and holidays. It’s a convenient and cost-effective option to see a provider while maintaining high-quality care.

Onsite provider solutions:

Healthcare support when you need it most.

PHP’s healthcare delivery model is robust enough to meet the personalized goals of long-term care facilities and their residents, yet nimble enough to evolve and innovate as the industry demands.

Our dedicated nurse practitioners are available to be onsite to support the healthcare needs of residents up to three days a week. Our collective goal is ultimately to become part of the facility, to support resident health goals, improve quality of life and control costs.
